Effective September 1, 2017, the DSU Student Health and Dental Plan has switched insurance providers and our carrier is now Green Shield Canada.

Coverage through Great-West Life Ended August 31, 2017
If you have claims from the past year that have not been submitted yet, they need to be submitted as soon as possible. Claims are eligible up to 15 months from the date of service.

Since the policy is no longer active, you can submit eligible claims from the 2016-17 benefit year only by submitting a paper claim. Paper claims must include original copies of your claim form, receipts and doctor's referral (if required). Remember to always keep copies for your records.
