Fall Application Period

August 1st to September 30th, 2026

Fall Academic Enrolment

Every student in an eligible program who has not previously opted out of the plan is charged the plan fee(s), and eligible to opt out by the specified fall deadline.

If this deadline is missed, you will be eligible to opt out next fall by the specified deadline as a returning student.

If you leave the school for more than a year you are considered a new student for the purposes of this form.

Winter Academic Enrolment

Every new student starting in the winter term who has been charged the plan fee(s) is eligible to opt out by the specified winter deadline.

If this deadline is missed, you will be eligible to opt out next fall by the specified deadline as a returning student.

If you leave the school for more than a year you are considered a new student for the purposes of this form.

Spring/Summer Academic Enrolment

Every new student starting in the spring/summer term who has been charged the plan fee(s) is eligible to opt out by the specified spring/summer deadline.

If this deadline is missed, you will be eligible to opt out next fall by the specified deadline as a returning student.

If you leave the school for more than a year you are considered a new student for the purposes of this form.

  1. The legal care program opt-out is annual. You will be re-enrolled next year on your enrolment anniversary. If you wish to opt-out again, you will need to complete this form before the applicable deadline.
  2. Students who opt-out of the legal care program will lose access to legal benefits.
  3. Approval of the legal care program opt-out will result in the OTSU Legal Care Program fee being refunded according to the school's policy.
  4. Once you opt-out of the legal care program, you cannot re-enrol until your enrolment anniversary.
  5. NO EXCEPTIONS will be made if the deadline is missed. Students are responsible for paying the plan's mandatory fees if they miss the opt-out deadline.
  6. Upon completing the legal care program opt-out form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting an opt-out request.
