Student Health & Wellness!mystudentplan is a benefits plan for students and can provide coverage for prescription drugs, vision care, dental care, mental health counseling, and more. The plan is designed to supplement provincial health insurance and provide additional access to medical services.

 

Submitting Claims

Have receipts for eligible expenses after the effective date of your plan? There are a couple of options to get reimbursed.

Online and Mobile Claims

If you have receipts for eligible expenses, there are a couple of options to get reimbursed. The online claims process is the quickest and easiest way to get your money back for the portion that is covered by the plan. To know the portion covered by the plan, please visit Plan Details.

Online Claims

The online claims process is the quickest and easiest way to get reimbursed. Canada Life provides an online hub that provides access to your group benefits plan. It is important to register on Canada Life once your plan is active. For new students, there are up to 60 days from the start date of the term for the enrollment process to be completed and for Canada Life to have you on file.

Through my.canadalife.com you can submit claims, check balances, investigate claim status and history, and set up Direct Deposit of claim payments by completing the Banking section under your Profile. Once this has been completed claims reimbursements will be deposited into your bank account and you will be notified by e-mail when a claim has been processed.

For direct billing at services such as vision care and paramedical services, update your complete address under the personal info tab under your Profile.

Registering with Canada Life:
  • Step 1: Open your Internet browser and go to my.canadalife.com
  • Step 2: Click on the Register button located under the Sign in red button
  • Step 3: Click on the Select button under the Benefits information
  • Step 4: Complete the form with the following information
    • Plan Number: 330753
    • Member ID: University of Calgary Student ID
  • Step 5: If you are found on the system, you will get a link to finish your registration. Click on the link and complete your registration
My Canada Life registration

You need to provide your banking information to get a direct deposit of your claim. Canada Life takes 2 business days to update that information and the system. Once the banking information is approved, you are able to submit your online claims.

Please note: You have 12 months from the date the eligible expense is incurred to submit online claims to Canada Life for consideration under your student plan.

Fast-track Online Claims

Online claims are the easiest and fastest way to get your claim processed. Use it for claims for:

  • Health
  • Vision
  • Drugs
Other Claims

Claim forms and supporting documents previously submitted by mail as a manual claim can now be uploaded online. Please allow extra time for processing. This method would be used to submit claim forms for the following claims:

  • Estimates of what we'll cover (predeterminations)
  • Health and drug claims that can't be submitted via fast-track online

Mobile Claims

If you prefer access to your benefits plan through your mobile, you can download and install the app on your android or apple device.

Manual Claim Forms

The manual claims process could take 4-6 weeks to get reimbursed. You must download and complete the appropriate form to submit a manual claim. Once all forms & supporting documents are completed, you need to mail in all information directly to Canada Life. If you have your banking information online with Canada Life, they will direct deposit the eligible portion. If you do not have your banking information included on your Canada Life account, a cheque will be mailed to you. Remember to keep a copy of all documents mailed for your records.

  • Health Claim Form: If your claim is related to eligible health services such as prescription drugs, ambulance services, paramedical, medical equipment, or vision expenses, download and complete the Health Claim Form
  • Dental Claim Form: There are no claim forms required to submit for the Managed Dental Plan. Claims are submitted through the Managed Dental Centre directly. You must visit the Managed Dental Provider in order to receive reimbursement for your dental care expenses.
  • Emergency Dental: If an emergency exam is required by a covered person, who at the time of incident and exam is more than eighty (80) kilometers away from Calgary, payment will be limited to that of an emergency exam and single periapical x-ray. The maximum amount payable will be $75 per covered person.

Once the appropriate form is completed, attach the original receipts and other documents to support your claim. Then mail the package to Canada Life to the address at the bottom of the claim form.

Pay-Direct Claims

Pay direct allows service providers to directly bill Canada Life for eligible expenses. Direct billing is dependent on each service provider’s billing practices.

For direct billing at vision care and paramedical providers, you must register with my.canadalife.com and update your address in the personal info section on your profile.

To access direct billing, students must show their Plan Card at the provider's office and just pay the portion that is not covered by the plan. If you do not have your Plan Card, you use the Plan Card Generator Tool

Pre-Authorization

A pre-authorization is when a form is submitted to Canada Life for a health care service, treatment plan, prescription drug, or medical equipment to pre-determine whether or not it is covered by the plan. Canada Life requires pre-authorization for certain services before you receive them, except in an emergency. When submitting a pre-authorization form, you will receive approval along with the amount that will be covered or an explanation of why it may have been denied.

To submit a pre-authorization form, you need to download the form according to the services that you need for approval and submit the completed form to Canada Life. The following are the forms:

  • Drug Exception Request Form: Download and complete the Drug Exception Request Form to request coverage of a drug that is not automatically covered under the drug plan. Before downloading and completing this form, please use our Prescription Drug Search Tool to find out if your medication is covered by the plan. This form must be completed by your physician. Email completed form to cldrug.services@canadalife.com
  • Drug Prior Authorization Form: Some prescription drug claims may be covered for specific conditions or circumstances only, and/or in pre-defined amounts and need to be approved before you can apply for reimbursement. Use the form to request coverage of a drug that is covered by the plan but has a note advising you that prior authorization is required after using the Prescription Drug Search Tool. Each prescription drug has a form, please download it from Canada Life Drug Prior Authorizations Forms. This form must be completed by your physician.
  • Health Claim Form: Download and complete the Health Claim Form to request coverage of medical equipment, appliances, and services covered by the plan. This form must be completed by the student and a physician's referral is required.

Once you have all sections of the form completed and signed, you can submit it online to Canada Life at https://my.canadalife.com under the Make a Claim and other claims section. You will be notified by email, and you will see the result on the my.canadalife.com website.

Policy Information

The insurance carrier for your student health plan is Canada Life. For any claims or coverage inquiries, students must contact Canada Life directly at 1-800-957-9777. You will require your plan number (330753) and student ID number when calling.

You are required to submit original copies of your manual claim form, receipts, and doctor's referral (if required) when sending in your manual claim to Canada Life by mail. Remember to always keep copies for your records.
