Policy Information

When accessing the Health and/or Dental claim form please use the below information as a guide to complete the required sections of the form:
  • Plan Number: 166175
  • Division Number: Not Applicable Leave Blank
  • Plan Name: Concordia Students' Association
  • Employee Identification Number: Your Student ID
  • Employee Name & Address: Your Name & Current Mailing Address
You are required to submit original copies of your claim form, receipts and doctor's referral (if required) when sending in your claim to the insurance carrier. Remember to always keep copies for your records.

Pay-direct claims

Pay-direct claims allow for direct billing of eligible costs between your pharmacy and/or dental office and the insurance company. Pharmacy claims are processed immediately so that students are not out of pocket the full expense at the time of purchase. Dental claims are processed based on the individual dental office's billing practices. These claims are accessed with the myBenefits Card.

GroupNet

Your Student Plan provides various options for claims submissions, including mobile claims, online claims, and provider e-claims. All these claims’ functions require you to register for GroupNet for plan members. GroupNet is the online claims solution provided by Canada Life. Registration can be completed online through a web browser or through the GroupNet Mobile app available on Android and Apple devices.

Once you have set up your account, you will then be notified by e-mail when a claim has been processed and that claim details are available for viewing online. You will be able to see any balances remaining (for coverages that have maximum amounts) or when you can go for your next appointment.

To register for GroupNet, please use the information below. Upon registering you will receive a verification email so you can sign into GroupNet to complete your profile:
  • Plan Number:166175
  • ID Number: Your Student ID
  • Plan Name: Concordia Students Association (CSA)
  • Name & Address: Your Name & Current Mailing Address (Required for some claims functions)
  • Banking Info: For direct deposit of any claims submitted online or via paper
Reminder: Make sure you provide your current mailing address by choosing “Manage mailing address” in the Profile section. A mailing address is required for provider e-claims.

Mobile Claim

With Canada Life's new GroupNet™ Mobile app, you can now access a variety of plan member services while you're on the go. Use your mobile digital device to quickly and easily:
  • Submit claims online with eClaims - part of Canada Life's industry-leading GroupNet online services.
  • Access personalized coverage information about benefits, claims and more.
  • Locate the nearest provider who has access to Provider eClaims, using a built-in GPS mapping tool.
To use GroupNet Mobile, you need to be registered for GroupNet for Plan Members. Registering is easy; just click GroupNet for Plan Members to begin the process.

You'll need your group benefits plan number and your plan member ID number. To use the app to submit claims, you must also be signed up for Direct Deposit of claim payments and eDetails (email or text notification when your claims are adjudicated).

GroupNet Mobile is free on Google Play, BlackBerry App World and the App Store.

Health Claim Form

To submit a manual claim, complete an insurance Health Claim Form, attach the original receipts and documents, and mail to the insurer. Remember to keep a copy of all original documents for your records.

Dental Claim Form
