RE-Enroll Applications

- Previously Opted-Out? You are/may be eligible to Re-enroll on your 1 year anniversary into your Health & Dental benefit plan


- If you want to Re-enroll in your Health & Dental benefits; please, contact SANQC Office "BEFORE" the start of your next Term.


- Contact SANQC Office for your current eligibility, deadline, and Application Process.

Application Deadline

14 days from the anniversary of starting your program

Make sure to submit your form within 14 days from the anniversary of starting your program as the applications received after the deadline will be denied. After the deadline, the online form is only available for students who have lost comparable coverage.

Re-Enroll Eligibility

(1) When students opted out of the Student Benefits Plan and are enrolled on the anniversary of the starting an eligible Program/Term (Contact SANQC For more details) and,

(2) When Students have lost comparable Health & Dental coverage.

How to Re-Enroll

- You must be currently opted out of the Student Benefits Plan.

- Meet the re-enroll eligibility Criteria (contact SANQC office for more details on your current eligibility & deadline)

- Complete the online Application Form before your application deadline

Things to consider:
  • Students can choose to re-enroll in the Health, dental or both plans on their plan anniversary OR significant change in circumstance.
  • NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
  • Approval of the Student Benefits Plan re-enrollment application will result in the Health Plan Fee and/or the Dental Plan Fee being assessed. You will receive an email informing you how to pay the fees. You can also reach out to SANQC office or office of the Register to know more about your fees payment.

Have you ever Opted Out of the Student Health and Dental Plan?

Is this current term the anniversary of opting out of the Student Benefits Plan?

The anniversary refers to the annual occurrence of when you opted out of the Student Benefits Plan. For instance, if you opted out in January, your anniversary of opting out would happen every January.

Have you lost your comparable health and dental Coverage?

Have you contacted SANQC Office for your eligibiility and Re-enrollment deadline?
