The plan is designed to supplement provincial health insurance and provide additional access to medical services.

Steps to successfully registering with Canada Life

Haven't registered yet? It's simple and it only takes a few minutes. Registering on the Canada Life website is easy, please complete the following steps.

Step 1. Open my.canadalife.ca website

Open a web browser on your computer or mobile device and go to my.canadalife.com, then click on the Register button located under the Sign In red button.

Click on the Select button under the Benefits information.

Savings and Both are not options for the Student Benefits.

Step 3. Let Canada Life find you in their system

Complete the online form with the following information:

  • Plan Number: 330760
  • Member ID: Bow Valley College Student ID number

Students are allowed to register on the Canada Life system once the validation period is completed. After 45 days from the official start date of your program.

If you get the following message "We couldn't find your file at the moment", that means the validation period is not completed yet or you are not eligible for the plan. Please contact your Benefits Plan Office at bowvalleyplan@mystudentplan.ca to confirm eligibility, enrollment, or validation period completed.

Please provide the same information that you used when you completed your registration on the school, your first name, last name, and date of birth must match your school's records.

Carefully read the instructions to create the password. If you do not read them and your password does not match the guidance, the system will show an error message requiring you to edit the password created.

Check the terms and conditions box and click on the Continue button. Please complete the information and make sure that you click on the Submit button to finish the online registration form.

To complete the registration with Canada Life, you must open an email sent by Canada Life and click on the link to Verify the ownership of your email address. If you do not click the link included in the email, your account will be not activated, and you will not have access to log in to the system.

To access your profile, please click on the circle with your initials at the top right corner of the website.

You must update the address and postal code in your profile to activate direct billing at vision care clinics and paramedical practitioners.

You must provide your banking information to activate the online claims and get direct deposits when submitting claims. Once the banking information is approved, you can submit your online claims.

Canada Life takes 2 business days to update that information on the system.

Mobile Access

Get access to your benefits information anywhere with the Canada Life mobile app. Please download and install the app on your Android or Apple device.

Online Claims

Your online account makes it easy to submit your claims online. The online claims process is the quickest and easiest way to get your refund for the portion that is covered by the plan. Use fast-track online Claims for Health, Dental, Vision, and Drugs.

Other Claims

Claim forms and supporting documents previously submitted by mail as a manual claim can now be uploaded online. Please allow extra time for processing. This method would be used to submit claim forms for the following claims:

  • Estimates of what we'll cover (predeterminations)
  • Health, dental, and drug claims that can't be submitted via fast-track online

How to submit online claims?

Step 1. Open the my.canadalife.ca website and log in to your account

Open a web browser on your computer or mobile device and go to my.canadalife.com, then provide your username and password and click on the Sign In red button.

If you have not registered with Canada Life, please follow the steps described in the Registering with Canada Life section.

On the main menu, click on the Make a Claim option, then click on the Start a Claim button.

Be sure to have your receipts and any supporting documents handy. Documentation such as prescriptions or doctors' referrals /notes are required for some benefits.

On the dropdown box, choose the person who the claim is for. If you have added your dependents to your plan, your dependents will be on the list. Otherwise, you only see the student's name.

You need to select one of the claims online forms to start, carefully read each option and click on the services or benefits that you are going to claim.

Please note that your plan may not include coverage for all the claim types shown on the image or your account.

You must complete the online form to provide the name, address, postal code, and other information required to process your claim. Make sure that provide the same information that you have in your receipt.

Then, you must anwer a few yes or not questions about the claim.

You must click on add the expense button and complete the form. The system shows the information that you need to complete.

You can include up to 7 expenses if from the same provider. Then, you must upload the receipt and other supporting documents to process your claim.

Please make sure that you click on the submit button to finish your claim.

Please note: You have 12 months from the date the eligible expense is incurred to submit online claims to Canada Life for consideration under your student plan.

Paper Claims

The manual claims process could take 4-6 weeks to get reimbursed. You must download and complete the appropriate form to submit a manual claim. Once all forms and supporting documents are completed, you need to mail in all information directly to Canada Life.

If you have your banking information online with Canada Life, they will direct deposit the eligible portion. If you do not have your banking information included on your Canada Life account, a cheque will be mailed to you.

Remember to keep a copy of all documents mailed for your records.

Start a paper claim

Use these forms to make paper claims. You'll find instructions for submitting each form on the form itself. Download the form and print it, or fill it out in Adobe Reader XI or higher (not your browser) and save it.

The following information is required on the claim form:

Health Claim Form

Use this form to make a claim for expenses such as prescriptions, vision care, ambulance services, paramedical services, or medical equipment.

Dental Claim Form

Use this form to make a claim for dental expenses. You'll need the dentist to fill in some sections.

Once the appropriate form is completed, attach the original receipts and other documents to support your claim. Then mail the package to Canada Life to the address at the bottom of the claim form or to the following address:

Please note: You have 15 months from the date the eligible expense is incurred to submit manual claims to Canada Life for consideration under your student plan.

Pay-Direct Claims

Pay direct allows service providers to directly bill Canada Life for eligible expenses. Direct billing is dependent on each service provider’s billing practices.

For direct billing at vision care and paramedical providers, you must register with my.canadalife.com and update your address in the personal info section on your profile.

To access direct billing, students must show their Plan Card at the provider's office and just pay the portion that is not covered by the plan. If you do not have your Plan Card, you must generate the Plan Card.

Pre-Authorization

A pre-authorization is when a form is submitted to Canada Life for a health care service, treatment plan, prescription drug, or medical equipment to pre-determine whether or not it is covered by the plan. Canada Life requires pre-authorization for certain services before you receive them, except in an emergency. When submitting a pre-authorization form, you will receive approval along with the amount that will be covered or an explanation of why it may have been denied.

To submit a pre-authorization form, you need to download the form according to the services that you need for approval and submit the completed form to Canada Life. The following are the forms:

Health Claim Form

Use this form to get an estimate for expenses such as paramedical services, medical equipment or other services covered by the plan. You'll need the physician to fill in some sections and physician's referral is required.

Dental Claim Form

Use this form to get an estimate for dental expenses exceeding $500.00. You'll need the dentist to fill in some sections or the dentist sends the form directly to Canada Life.

Drug Exception Request Form

Use this form to request coverage of a drug that is not automatically covered under the drug plan. conditions Apply.

Drug Prior Authorization Form

Use this form to request coverage of a drug that is covered by the plan but has a note advising you that prior authorization is required after using the Prescription Drug Search Tool.

Once you have all sections of the form completed and signed, you can submit it online to Canada Life at https://my.canadalife.com under the Make a Claim and other claims section. You will be notified by email, and you will see the result on the my.canadalife.com website.

If you prefer a manual claim, please mail your form to Canada Life at the following address. You will be notified by mail.

Policy Information

The insurance carrier for your student health & dental plan is Canada Life. For any claims or coverage inquiries, students must contact Canada Life directly at 1-800-957-9777. You will require your plan number (330760) and student ID number when calling.

You are required to submit original copies of your manual claim form, receipts, and doctor's referral (if required) when sending in your manual claim to Canada Life by mail. Remember to always keep copies for your records.
