Submission of an online form indicates:

• an understanding that it is solely the responsibility of the student whose name and identification number (The Student) is on the form to ensure that the form has been received by the benefit office/representative.
• an understanding that the information provided is required in order fulfill the purpose of the form;
• authorization and consent to the use, release and exchange of said information between the educational institution, the student organization, the plan broker, third party service providers and the insurance company(s) to be used solely in connection with the administration of the Student Benefits Plan;
• confirmation that all the information provided is accurate; and
• that additional terms as indicated below by type of form also apply.

Falsification of any information provided by The Student is a serious form of fraud.

All eligible students enrolled in the legal care program may submit an online application to opt-out of the legal care program based on their personal choice at the start of their academic year before the applicable deadline.

Students who opt-out of the Legal Care Program will lose access to legal benefits.

You cannot opt-out of coverage at any point during the school year except your program start date anniversary.

NO EXCEPTIONS will be made if the deadline is missed. Students are responsible for paying the Legal Care Program plan's mandatory fee if they miss the opt-out deadline.

OPT-OUT APPLICATION: The Student wishes to decline the Legal Care Program coverage.

To opt out of the Legal Care program, student must submit an online Legal Care opt out application. The student acknowledges that because of the opt-out, they forfeit all rights to coverage otherwise available to them under the Legal Care Program. The student realizes that they will not be able to rejoin the Legal Care program until the policy anniversary.

OPT-OUT DEADLINE: Students must submit an online Legal Care opt out application at the start of their academic year when they have been assessed the Legal Care fee on their student account.

OPT-OUT EMAIL CONFIRMATION: You will receive a reply email confirmation after you complete and submit the online Legal Care opt-out. Please retain a copy of the email confirmation for your records. The email confirmation is your ONLY proof that you applied to opt-out of the coverage. If you do not receive a confirmation email, please contact the Student Benefits Plan Office before the applicable deadline.

FAMILY: The Legal Care fee provides coverage for the student only and family members do not receive Legal Care coverage.
