• Previously opted out of the Student Health and Dental Plan; and
• Meet the eligibility criteria to be enrolled in the Student Benefits Plan; and
• Submit the online application to enroll before the applicable fall deadline.

You must provide documentation such as a letter from the alternate insurance carrier or the employer stating the student's name, insurance carrier's name and policy number and the loss of coverage date by emailing georgianplan@mystudentplan.ca.

Eligible students who miss the 30 days deadline would be able to enroll the upcoming fall term.

• Must be considered full-time equivalent (between 40-66% course load) due to a persistent, prolonged, or permanent disability documented with Student Accessibility Services; and
• Must email the full-time equivalent confirmation letter to georgianplan@mystudentplan.ca. Ask your Student Accessibility Advisor to email the letter to the Student Plan Support Office; and
• Submit the online application to enroll before the applicable term deadline.

The Student Benefits Plan fee must be assessed during the first semester of your academic year. Each year, you are required to complete the opt-in form during the applicable term deadline. No exceptions can be made.

If you are taking a full-time course load, you will automatically be assessed the fee and enrolled in the Student Benefits Plan. You do not need to complete this online form.
