Re-Enroll Applications

Submit your health and dental re-enroll application before the deadline.

Application Deadline

Within 30 days from the loss of coverage date

Make sure to submit your form within 30 days of losing your previous coverage as the applications received after the deadline will be denied. The online form is only available for students who have lost comparable coverage.

  1. All eligible students are automatically enrolled in the Student Health and Dental Plan and do not need to submit a re-enroll form. You will see the TMGSU Health and Dental Plan Fee on your student account to confirm you are enrolled.
  2. Students must apply to re-enroll to both the Health and Dental Plans, as there are no health plan or dental plan enrollment options.
  3. Approval of the Student Benefits Plan enrollment application will result in the TMGSU Health Plan Fall/Winter GS being assessed. You will receive an email informing you how to pay the fees.
  4. NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
