Family Coverage

To add family coverage to your International Student Emergency Medical plan you must submit the completed Family Add-On Form, along with the required fees to the CNC Student Benefits Plan Office before the established deadline.

International student dependents must be in Canada at the time of application, fees for dependents are determined separately from individual student fees assessed with your tuition and are paid separately to the plan's in-house platform. Dependent coverage will be for the same duration as the student member.

2022 - 2023 Family Add-On Fees

Example
If you would like to add 2 dependents (Spouse and child), the fee is $120.00 per each dependent. The total amount of your family add on fees will amount to $240.00 ($120.00 * 2).
  • The fee is not included in your tuition, and fees are collected upon receipt of a confirmation approval that provides you with a link to make the appropriate payment as confirmed by the CNC International Student Benefit Plan Office by credit card on our website.
  • The fee covers 90 days from the effective date of the plan.
  • The fees for Family Add-on are non-refundable.

Definition of Dependent on International Student Emergency Plan

  • The spouse or common-law spouse (including same-sex) of a primary Insured Person (but excluding those legally separated), and under the age of sixty five (65).
  • Unmarried children, step-children, foster children, and legally adopted children, who are Dependent on the insured person for support, provided that such children are not less than fifteen (15) days old (unless the birth of a newborn is an insured event under this policy in which case the newborn is insured under Maternity Benefit to the 15th day of birth) and under the age of nineteen (19) years old (or under the age of twenty-five (25) years old provided it can be proven that the child is continuing in full-time education).
  • Unmarried children, step-children, foster children, and legally adopted children, who are dependent on the primary Insured Person for support due to physical or mental disability.

To be eligible for coverage Dependents must be accompanying the Primary Insured Person (you the international student) in Canada.

Please read these terms and conditions carefully before submitting online applications.

Submission of an online form indicates:

  • an understanding that it is solely the responsibility of the student whose name and identification number (The Student) is on the form to ensure that the form has been received by the benefit office/representative;
  • an understanding that the information provided is required in order fulfill the purpose of the form;
  • authorization and consent to the use, release and exchange of said information between the educational institution, the student organization, the plan broker, third party service providers and the insurance company(s) to be used solely in connection with the administration of the insurance Plans;
  • confirmation that all the information provided is accurate; and
  • that additional terms as indicated below by type of form also apply

Falsification of any information provided by The Student is an act of fraud.

INTERNATIONAL STUDENT EMERGENCY PLAN OPT-OUT FORM: The International Student Emergency Medical Insurance Plan is mandatory, there is NO opt-out option.

INTERNATIONAL STUDENT EMERGENCY PLAN FAMILY ENROLMENT FORM: The Student understands that in order to enroll family on the emergency plan, The Student must be enrolled in this plan. Your family can only be covered while you are a student on this plan(s). A Family Add-on must be submitted by the applicable deadline. The deadline will be provided to The Student by the CNC Student Benefits Plan Office. The Student understands that the family coverage provided to the plans will need to be renewed each Benefit Year. Family add-on fees are non-refundable.

The Student understands that the information requested is required to provide the same coverage received as a student to spouse and/or dependent(s). The Student has the authority to provide such information and authorizes the use of the information provided where it is required in the administration of the insurance benefits. The Student understands that in order to receive coverage for an eligible spouse or dependent(s) the appropriate fee must be paid. The Student agrees to pay the appropriate fees for the coverage requested prior to the applicable deadline.

The Student confirms that the individual(s) for whom coverage is being requested qualify based on the eligible definitions of spouse or dependent.

Definition of Spouse: Spouse means the person who is a resident of Canada, and who is married to The Student, or a person of either sex who has continuously co-habitated with The Student for a period of at least 12 months and who is publicly represented as The Student's wife or husband.

Definition of Dependent(s): Dependent means an unmarried child who is a resident of Canada, and entirely dependent on The Student for maintenance and support, and who is:

  1. under 21 years of age, CONFIRM.
  2. under 25 years of age and attending a college or university full-time, or
  3. physically or mentally incapable of self-support and became incapable to that extent while entirely dependent on The Student for maintenance and support and while eligible under 1) or 2) above.

Family Enrolment is not guaranteed by submitting this form; the applicant must be eligible for the program and the appropriate fees must be paid.
