Making a Claim



Policy Information

When accessing the Health and/or Dental claim form please use the below information as a guide to complete the required sections of the form:
  • Plan Number: 59355
  • Division Number: Not Applicable Leave Blank
  • Plan Name: Students' Association of MacEwan
  • Employee Identification Number: Your Student ID
  • Employee Name & Address: Your Name & Current Mailing Address
You are required to submit original copies of your claim form, receipts and doctor's referral (if required) when sending in your claim to the insurance carrier. Remember to always keep copies for your records.

Please note: You have 15 months from the date the eligible expense is incurred to submit claims to Great-West Life for consideration under your student plan.

Pay-direct claims

Pay-direct claims allow for direct billing of eligible costs between your pharmacy and/or dental office and the insurance company. Pharmacy claims are processed immediately so that students are not out of pocket the full expense at the time of purchase. Dental claims are processed based the individual dental offices billing practices. These claims are accessed with the myBenefits Card.

GroupNet

Effective September 2018, Great-West Life is providing a new online experience through a new Groupnet*. To take advantage of the new GroupNet, visit gwl.greatwestlife.com. If you were registered under the old GroupNet, your information has been transferred and can be accessed through the new GroupNet service.

Through GroupNet you can submit claims, check balances, investigate claim status and history, and setup Direct Deposit of claim payments by completing the eDetails section. Once you’ve set up your account, you will then be notified by e-mail when a claim has been processed and that claim details are available for viewing online.

* If you are a member on more than one policy with Great-West Life, you are currently only able to access one policy through the new Groupnet portal. You can still access multiple policies through the old Groupnet portal, which can be found by clicking here.

Mobile Claim

With Great-West Life's new GroupNet™ Mobile app, you can now access a variety of plan member services while you're on the go. Use your mobile digital device to quickly and easily:
  • Submit claims online with eClaims - part of Great-West's industry-leading GroupNet online services.
  • Access personalized coverage information about benefits, claims and more.
  • Locate the nearest provider who has access to Provider eClaims, using a built-in GPS mapping tool.
To use GroupNet Mobile, you need to be registered for GroupNet for Plan Members. Registering is easy; just click GroupNet for Plan Members to begin the process.

You'll need your group benefits plan number and your plan member ID number. To use the app to submit claims, you must also be signed up for Direct Deposit of claim payments and eDetails (email or text notification when your claims are adjudicated).

GroupNet Mobile is free on Google Play, BlackBerry App World and the App Store.

Health Claim Form

To submit a manual claim, complete an insurance Health Claim Form, attach the original receipts and documents, and mail to the insurer. Remember to keep a copy of all original documents for your records.

Employee Identification Number: Your Student ID

Dental Claim Form
