Online Forms

All new eligible students are automatically enrolled in the Balanced Health and Dental Plan. Students have one opportunity at the start of their academic year to submit online forms before the applicable deadline. Students must read and agree to the Terms and Conditions agreement before completing the forms.

Due to the high volume of online applications, it may take a few minutes for the following pages to load after clicking the "Next" or "Submit" buttons. Thank you for your patience.

Our forms will be open on August 1, 2022 and the deadline to submit is September 29, 2022.

Individual Insurance

Are you not eligible for the Student Plan?

If you are not automatically enrolled in the student plan or have missed the enrolment period to add family to the student plan, there are personal insurance solutions available.

Our personal insurance solutions fit the needs of individuals who are not eligible for the extended health and dental coverage through a Student Plan. This may apply to you because you may not be included through the established group plan eligibility criteria, or you may be changing enrolment status, graduating, or leaving school. There are many coverage and enrolment options at cost effective rates, including options that do not require any medical questionnaires or exams to qualify for coverage.

Personal Insurance Scenarios

  • I am losing mystudentplan
    I am graduating or leaving school and want information on purchasing personal insurance.
  • I don’t have mystudentplan
    I am not eligible to have mystudentplan and want information on purchasing personal insurance. This option also applies to students who missed the opt-in deadline for their student plan.
  • I want to cover my family
    I missed the deadline to add family to mystudentplan and want information on purchasing personal insurance for my family.
  • I have other insurance & financial needs
    I have other personal insurance needs outside the previous options listed and want to discuss them with an advisor. This option also applies to individuals looking for travel insurance.

The personal insurance solutions are offered in partnership with Canada Life through their PlanDirect™ individual insurance products.

Flex your Health and Dental Plan  

Fall 2022 Deadline: September 29, 2022 

All new eligible students are automatically enrolled in the Balanced Plan, you do not need to complete this form if you want to remain in the Balanced Plan.

Important notes:
  • Students can Flex their benefit plan only once at the start of their academic year. Once their Flex plan is approved, it will remain in effect for minimum 2 year.
  • There is a 60 day validation period from the start of the term. Your flex plan option will be not reflected on your account until the end of the validation period.  
  • If the student extends the plan to the family, the family will have the same plan that the student has chosen. 
  • Students choosing to Flex their Benefits must be enrolled in BOTH the Health and the Dental plan. 
  • NO EXCEPTIONS will be made if the deadline is missed.
  • Once you complete the Flex your Plan form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting a Flex your Plan request.

Health and Dental Opt-In

Fall 2022 Deadline: September 29, 2022

All new eligible students are automatically enrolled in the
Balanced Health and Dental plan so only submit an opt in form
if you have previously opted out. 

There are 3 scenarios to re-enroll into the Student Health and Dental Plan:

  1. Program start date: All eligible full-time students can re-enroll at the start of their academic year before the applicable deadline:
    • Fall enrollment: Students starting in September have fall deadline
    • Winter enrollment: Students starting in January have a winter deadline
  2. Loss of comparable coverage: Students who have lost their comparable insurance coverage may apply within 30 days of losing that coverage. You must provide the proper documentation and complete the online form. Documentation includes a letter from the insurance carrier or the employer stating the students' name, loss of coverage date, the carrier name, and policy number. This documentation must be emailed to macewanplan@mystudentplan.ca
  3. Lifestyle Change: There are certain circumstances known as lifestyle changes that allow a student to make changes to their enrollment status within 30 days of the change date. Lifestyle changes include getting married, meeting the twelve-month co-habitation requirement for common law status, birth/adoption of a child and meeting provincial plan residency requirements. You must provide the proper documentation and complete the online form. Documentation includes a marriage certificate, birth certificate or adoption documents. This documentation must be emailed to macewanplan@mystudentplan.ca
Important note:
  • All eligible students are automatically enrolled in the Student Health and Dental Plan and do not need to submit an opt in form.  You will see a health fee and dental fee on your student account to confirm you are enrolled.
  • There is a 60 day validation period from the start of the term. During this time, you would have to pay out of pocket for all eligible Health and Dental expenses and keep the receipts. After the enrollment process is complete, you will be able to submit online claims for reimbursement  and your benefits card will be automatically activated.
  • NO EXCEPTIONS will be made if the deadline is missed. It is the student's responsibility to pay the mandatory plan fees once there are added to the student account.
  • Once you complete the Opt-In form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting a Opt-In request.

Health and Dental Opt-Out 

Fall 2022 Deadline: September 29, 2022

Things to consider when thinking of opting out:

  • A parents insurance plan will stop covering you if you are a part-time student over the age of 21 or a full-time student over the age of 25.
  • You can coordinate 2 benefit plans and increase your coverage up to 100%! Find out more here: Coordination of Benefits
  • What is in the best interest of my overall health & wellbeing?
  • Does my existing coverage, meet all of my needs?
  • Only time you can get back on this student plan is at the start of next year OR within 30 days of losing your alternate coverage.
Important notes:
  • Alberta Health Care is not acceptable as comparable insurance coverage.
  • The student plan is not acceptable as comparable insurance coverage.
  • Students with comparable coverage can choose to opt out of health, dental or both. 
  • The comparable coverage accepted could be from parents, spouse, work, government, or band.
  • Approval of the Students opt-out will result in the plan fee being refunded according to the school's policy.
  • The Opt-Out result is a one-time process. This means students do not need to apply every term or year. 
  • Please be advised, random audits are done, and you may be asked to show proof of alternate coverage.
  • NO EXCEPTIONS will be made if the deadline is missed. It is the student's responsibility to pay the plan mandatory fees, should they miss the applicable opt-out deadline.
  • Once you complete the opt out form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting an opt out request.

Health Plan: Do you wish to Opt-Out of this portion of the Student Benefit Plan?

Family Add On 

Fall 2022 Deadline: September 29, 2022

The fees on the student account are for the student only.
Students must pay the additional fees before the deadline to have their family members added onto the plan.

  • These fees are not included in your tuition, and you will pay them with a credit card on our website if your application is approved. 
  • The fees for Family Add-on are non-refundable.

Eligible dependents

Students can add the following dependents to their plan:

  • Spouse:  By marriage or under any other formal union recognized by law, or your common-law spouse of either sex who has cohabitated with the student minimum for one year. 
  • Children: Your natural children, stepchildren, legally adopted children, and children for whom you/and or your spouse have been appointed as guardian(s) for all purposed pursuant to an Order of the Court.
    • Unmarried
    • Under 21 years of age and not employed full-time
    • Under 25 years of age, if they are attending a college or university full-time, or
    • Physically or mentally incapable of self-support and became incapable to that extent while entirely dependent on the student for maintenance and support and while eligible under 1) or 2) above
 
Important notes:
  • You can only extend your plan to your spouse or partner and your children. Parents, Grandparents, Siblings are not eligible dependents to extend your plan. 
  • The family would be covered by the same plan the student has. If the student is enrolled in the Balanced Plan, the family would be in the Balanced Plan.
  • The Family would be covered for one year. The student needs to keep full-time status during two consecutive terms. If the student loses the coverage, the family loses the coverage too.
  • Family Add On requests & fees are not automatically renewed must be submitted/paid every year.
  • There is a 60 day validation period from the start of the term. During this time, you would have to pay out of pocket for all eligible Health and Dental expenses and keep the receipts. After the enrollment process is complete, you will be able to submit online claims for reimbursement and your benefits card will be automatically activated.
  • Each family member can generate their own benefits card and included the student ID.  
  • The fees for Family Add-on are non-refundable.
  • NO EXCEPTIONS will be made if the application or payment deadline is missed.  
  • Once you complete the Family add-on form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting a family add-on request.

If your application is approved, you will receive a link to pay the applicable fees with your credit card and the deadline to pay them.
